BBL / Buttock Lift in Turkey, the process, the healing, and what I wish I’d known
The Brazilian butt lift is the procedure I thought longest about whether to support at all. It has the highest death rate of any cosmetic operation, and several British women have died having it in Turkey in recent years, their names have been in coroners' courts and on the news. This page is therefore different from the others: it starts with the risk.
My connection to this surgeon
I was a patient of the specialist I refer you to. I was so convinced by his work that I chose to work with him. On site, his clinic team looks after you, English-speaking, from the airport to the last check-up. He speaks English himself; your consent conversation happens without an interpreter. For legal reasons I give his name only in conversation.
What a BBL is
Fat is suctioned from the abdomen, flanks or back, processed and injected into the buttocks. Some of it is reabsorbed in the first months; how much varies. Alternatively or in addition there's a buttock lift with skin removal, usually after major weight loss.
The decisive point: fat must only be placed in the subcutaneous layer, never in or under the gluteal muscle. Large veins run there; fat entering them can cause a fat embolism that kills within minutes. Professional bodies including BAAPS require subcutaneous-only technique, ideally with ultrasound guidance.
Who it suits, and who it doesn’t
Suitable only with enough donor fat, stable weight, excellent health, and only with a specialist surgeon who demonstrably uses subcutaneous technique.
Not: with little body fat, clotting disorders, nicotine, in combination with many other procedures in one session. And not with any provider who plays down the risk.
The technique decides your life
Subcutaneous, the only defensible technique
Fat is placed exclusively in the layer under the skin, above the muscle fascia. Professional bodies worldwide, ISAPS, ASPS and BAAPS among them, have demanded this explicitly since the deaths. Many surgeons check cannula position with real-time ultrasound. Ask precisely: “Do you inject subcutaneously only, and do you check with ultrasound?” An evasive answer is a no.
Intramuscular, life-threatening
Fat in or under the gluteal muscle can enter the large gluteal veins and cause a fat embolism in the lungs or heart. That is the cause of the documented deaths, including British women whose cases went to inquest. Intramuscular placement used to be common because more fat survives there. No volume advantage justifies this risk.
Cannula size, angle, pressure
Larger cannulas, a flat angle, low pressure, injection only on withdrawal, those are the technical rules that lower the risk. You don’t need to understand them; you need to know they exist, and have a surgeon who names them unprompted.
Donor fat and volume
Not every woman has enough fat for a BBL. If you’re slim you can’t give much, and gaining weight before surgery to have more donor fat is medically nonsensical. Of the injected volume, a third to a half is reabsorbed; what’s there at three months largely stays. Want too much at once and you risk fat necrosis and cysts.
Buttock lift as an alternative
After massive weight loss the problem is often not volume but excess skin. Then a buttock lift with skin removal helps, with a scar across the upper buttock, but without the embolism risk of fat transfer. Sometimes both are combined.
Why I guide differently for a BBL
For a BBL I ask about technique before anything else. I only guide to a specialist who works subcutaneously with guidance, and I advise against it when donor fat is lacking, when other procedures are planned in the same session, or when the risk gets played down in conversation. That costs me referrals. It’s the procedure where I accept that most readily.
The process in Turkey
Before you travel
Blood tests, clotting screen, medication list, stop smoking. Buy a BBL pillow and compression garments.
Arrival
The day before surgery.
Surgery day
Consent, ask explicitly about technique and ultrasound guidance. General anaesthetic, two to four hours. One to two monitored nights.
Days 1-7
No sitting on the buttocks; lie on your front or side. Compression, check-ups, drains at the liposuction sites.
Clearance and flight home
Usually after about a week, and only once the surgeon clears you. On the plane: sit only on the BBL pillow, get up regularly.
Healing week by week
The part clinic websites cover in one sentence, and the most important.
Pain at donor and recipient sites, swelling, bruising. Sleep on your front. Sit only to use the toilet.
Short periods of sitting on a pillow. Desk work limited.
Normal sitting once cleared. Compression continues.
Exercise once cleared.
Some fat has been reabsorbed. What's there now largely stays.
Final result.
What nobody likes to say
- People have died having this operation. That isn't scare-mongering from the other side, it's documented, including UK inquests.
- Not being allowed to sit for two to three weeks changes your daily life more than you think.
- Some of the fat, often a third or more, is reabsorbed. The result at four weeks isn't the result.
- If you lose weight, your buttocks shrink too.
- I only support a BBL after a conversation about technique, and I advise against it more often than for it.
Risks
General surgical risks: bleeding, infection, delayed wound healing, blood clots (DVT and pulmonary embolism), anaesthetic complications. Procedure-specific and especially serious: fatal fat embolism if fat is placed in or under the muscle. Also: fat necrosis with lumps and cysts, infection, asymmetry, seromas at donor sites, contour irregularities, marked volume loss. Professional bodies classify the BBL as the highest-risk cosmetic procedure.

Aftercare at home
In addition to general aftercare: sudden breathlessness, chest pain or confusion in the first days and weeks means call 999 (UK) or 112 (Ireland) immediately, that applies after you're home too. Keep to the sitting restriction and pillow strictly. And any doctor at home should know you've had a BBL if you present with other symptoms.

What a quote should contain
I give no prices here. Not because I don’t know them, but because any figure without an examination is misleading, and because this site also serves countries where price advertising for cosmetic surgery is restricted. A reputable quote for a BBL is itemised by surgeon, anaesthetic, hospital stay, materials and aftercare, and it states what isn’t included. When two quotes differ widely, it’s almost always the surgeon’s experience, the materials, the hospital or the length of stay. I’ll walk you through the clinic’s actual quote line by line.
Common questions
Why is the BBL so dangerous?
Because fat placed in or under the gluteal muscle can enter large veins and cause a fat embolism. Only subcutaneous placement is defensible.
How do I recognise a safe surgeon?
They name subcutaneous technique and ultrasound guidance unprompted, hold a verifiable specialist qualification, and refuse combinations with many other procedures.
How long can’t I sit?
Two to three weeks not directly on the buttocks, then with a pillow.
How much fat stays?
Individually variable. Often about two thirds. You see the result at three to six months.
What if I don’t have enough fat?
Then a BBL isn’t sensible. Gaining weight for the operation is medically nonsensical.
Do you guide BBLs at all?
Only after a conversation about technique and with a specialist who works subcutaneously with guidance. I advise against more often than for.
How long in Turkey?
Usually one week. The surgeon clears the flight home. The flight home is particularly unpleasant after a BBL, pillow only.
Can I combine a BBL with a tummy tuck?
That raises anaesthetic time and risk considerably. Reputable surgeons are very cautious here.
How long am I off work?
Desk work limited from week two to three because of the sitting restriction; exercise after six to eight weeks.
What does a BBL cost in Turkey?
It depends on donor areas, surgeon and hospital. I give no figures. And: a strikingly cheap BBL is the biggest warning sign of all.
What’s the alternative?
With excess skin a buttock lift; with a small volume wish sometimes doing nothing. We discuss both.
