Breast Implant Exchange in Turkey, the process, the healing, and what I wish I’d known

The implant exchange is the procedure nobody mentions the first time round, and one almost every woman with implants eventually faces. It's often more complex than the first operation because scar tissue, the capsule and changed anatomy come into play.

What an implant exchange is

The old implants are removed and replaced, or removed permanently, often combined with a lift. With capsular contracture the hardened capsule is partly or fully removed (capsulectomy). Sometimes the implant pocket is changed.

Reasons for exchange: capsular contracture, rupture or displacement, size wish, age of the implants, changes after pregnancy or weight fluctuation.

Who it suits, and who it doesn’t

Suitable for all of the above, an up-to-date scan (ultrasound or MRI) and the records of the first operation are essential.

Caution: without an implant card and operation report the surgeon is operating blind. If BIA-ALCL is suspected, get it assessed at home before any trip abroad.

The reasons and what follows from them

Capsular contracture

The commonest cause. Grades I and II are barely noticeable and not a reason to operate. Grade III (firm, distorted) and IV (painful) favour exchange with capsule removal. Swapping the implant but leaving the capsule carries a high recurrence risk.

Rupture

Modern cohesive-gel implants usually keep their shape when ruptured, which is why ruptures are often found only on ultrasound or MRI. A rupture is a reason to exchange, not an emergency.

Displacement, rotation, rippling

Anatomical implants can rotate, implants can migrate outwards or downwards, and with thin skin folds become visible. At exchange the pocket is adjusted or the plane changed.

Size wish or changed breast

After pregnancies or weight changes the old implant no longer fits. Exchange is then often combined with a lift.

Explantation

Permanent removal without replacement. Usually with a lift because the skin is stretched. Some women choose fat transfer as partial compensation. Honestly: the breast will be smaller and softer than with implants; the result takes months.

Capsulectomy: partial or total

With contracture the capsule is partly or fully removed. Total (en bloc, with the implant) is more demanding and required where BIA-ALCL is suspected. With an unremarkable capsule, partial removal often suffices. The capsule should be analysed histologically, ask for the report.

What you must bring

Without these the surgeon is operating blind. If the first clinic no longer has them, an MRI matters all the more.

The process in Turkey

Before you travel

Current ultrasound or MRI, records of the first surgery, implant card, blood tests. Stop smoking.

Arrival

The day before surgery.

Surgery day

Consent with imaging, general anaesthetic, one to three hours depending on capsule work. One night.

Days 1-6

Sports bra, drains if needed, check-up.

Clearance and flight home

Usually after about a week, and only once the surgeon clears you.

Healing week by week

The part clinic websites cover in one sentence, and the most important.

Week 1

Similar to the first operation; more swelling if the capsule was removed.

Week 2

Desk work possible.

Weeks 3-6

New implants settle. No exercise.

Week 6

Exercise once cleared.

Months 3-6

Final shape.

Yearly

Ultrasound check. Keep the new implant card.

What nobody likes to say

  • An exchange is often more involved than the first operation. More scar tissue, more unknowns.
  • After capsular contracture it can come back.
  • If you have implants removed for good you'll usually need a lift, and will have less volume.
  • Without the records of the first surgery, this is a blind flight.
  • The new implants won't last forever either.

Risks

General surgical risks: bleeding, infection, delayed wound healing, blood clots (DVT and pulmonary embolism), anaesthetic complications. Procedure-specific: recurrent capsular contracture, asymmetry, distortion from scarred tissue, sensory changes, wound problems along old scars, and with capsulectomy rarely injury to the chest wall.

Two risks are particular to going abroad: combining several procedures under one long anaesthetic, and the flight home. Both belong in the surgeon’s consultation, not the quote. In full: Cosmetic surgery abroad.

Aftercare at home

In addition to general aftercare: new implant card, operation report with details of the removed capsule, and, if the capsule was analysed, the histology report. Yearly ultrasound at home.

What a quote should contain

I give no prices here. Not because I don’t know them, but because any figure without an examination is misleading, and because this site also serves countries where price advertising for cosmetic surgery is restricted. A reputable quote for an implant exchange is itemised by surgeon, anaesthetic, hospital stay, materials and aftercare, and it states what isn’t included. When two quotes differ widely, it’s almost always the surgeon’s experience, the materials, the hospital or the length of stay. I’ll walk you through the clinic’s actual quote line by line.

Common questions

When does an implant need replacing?

With grade III-IV contracture, rupture, displacement or symptoms, not on a fixed date.

What is capsular contracture?

The natural tissue capsule around the implant hardens and tightens. The breast becomes firm, distorts, can hurt.

Does the capsule always have to come out?

With contracture yes, or it recurs. With an unremarkable capsule not necessarily.

Can I just have the implants removed?

Yes. A lift is usually needed because the skin has been stretched.

How long in Turkey?

Usually one week. The surgeon clears the flight home.

Is exchange worse than the first operation?

Often more involved because scar tissue and capsule come into play. Recovery is similar, slightly longer with capsulectomy.

Can contracture come back?

Yes. The risk is lower with capsule removal and a new plane, but not zero.

What documents do I need?

Implant card, operation report from the first surgery, current imaging.

What does implant exchange cost in Turkey?

It depends on whether the capsule is removed and whether a lift is added. I give no figures.

How often do I need checks afterwards?
