Facial Fat Transfer in Turkey, the process, the healing, and what I wish I’d known
With the years the face loses not only firmness but volume, cheeks flatten, temples hollow, the eye area looks tired. Fat transfer replaces that with your own tissue. The catch: some of it is reabsorbed, and nobody knows in advance how much.
My connection to this surgeon
I was a patient of the specialist I refer you to. I was so convinced by his work that I chose to work with him. On site, his clinic team looks after you, English-speaking, from the airport to the last check-up. He speaks English himself; your consent conversation happens without an interpreter. For legal reasons I give his name only in conversation.
What facial fat transfer is
Fat is gently suctioned from the abdomen, thighs or flanks, processed and injected with fine cannulas into cheeks, temples, tear troughs, chin or nasolabial folds. Very finely processed fat (micro- or nanofat) is used for thin skin areas and to improve skin quality.
Often combined with a facelift: the lift tightens, the fat fills. Together the result looks more natural than either alone.
Who it suits, and who it doesn’t
Suitable for volume loss in the mid-face, hollow temples and tear troughs, with enough donor fat and stable weight.
Not: for very slim women with no donor fat; with marked weight fluctuation, the transferred fat behaves like the rest of your body fat; with nicotine; if you expect a precisely predictable result.
Fat transfer or filler?
Both fill, but they are fundamentally different.
- Fat transfer: your own tissue, no foreign material, what survives is permanent. But: an operation, weeks of swelling, a result that isn't precisely predictable, partial reabsorption.
- Fillers: done in minutes, immediately visible, precisely controllable, but temporary and foreign material. No reason to travel to Turkey; you can get that at home.
- Combination: some surgeons fill broad areas with fat and refine fine areas with filler.
What micro- and nanofat mean
Finely processed fat with a high proportion of regenerative cells. It fills less but improves skin quality, used for fine lines, dark circles or scars. A brand or device name is not a mark of quality; the surgeon's experience is.
If you have no donor fat
Very slim women often have too little fat to harvest. Gaining weight for surgery is medically pointless and the fat behaves accordingly afterwards. Then fillers or a lift are the honest answer.
The right time
- Stable weight, facial fat behaves like body fat.
- Enough donor fat available.
- Smoke-free for several weeks before and after.
- Two weeks with no important engagements while you look swollen.
The process in Turkey
Before you travel
Blood tests, no blood thinners, stop smoking.
Arrival
The day before surgery.
Surgery day
Markings sitting up, consent, general or sedation. Harvest, processing, injection. One to two hours.
Days 1-5
Marked facial swelling and bruising at the donor site. Cooling, head elevated. Check-up.
Clearance and flight home
Usually after about a week, and only once the surgeon clears you.
Healing week by week
The part clinic websites cover in one sentence, and the most important.
Heavy swelling, the face looks overfilled. Normal, and it alarms almost every woman.
Swelling recedes markedly; concealable with make-up. Donor site still bruised.
The face gradually looks like you again. No exercise until cleared.
Some fat has been reabsorbed. Now you see what stays.
Final result. What's there now is largely permanent.
What nobody likes to say
- In the first week you look overfilled. Almost every woman regrets it on day three.
- Between a third and a half of the fat is reabsorbed. How much is not predictable.
- A second session is sometimes needed, not a failure, just normal.
- If you lose a lot of weight, the face thins again too.
- Small lumps or irregularities are possible, especially in thin skin such as under the eyes.
Risks
General surgical risks: bleeding, infection, delayed wound healing, blood clots (DVT and pulmonary embolism), anaesthetic complications. Procedure-specific: unpredictable volume loss, asymmetry, palpable lumps and hardening (fat necrosis), cysts, over-correction, irregularities at the donor site, and very rarely vessel occlusion if injected into a blood vessel, which is why this belongs solely with an experienced specialist.
Two risks are particular to going abroad: combining several procedures under one long anaesthetic, and the flight home. Both belong in the surgeon’s consultation, not the quote. In full: Cosmetic surgery abroad.
Aftercare at home
In addition to general aftercare: no pressure on the treated areas for the first weeks, no facial massage, don't sleep on your face. Judge the result no earlier than three months, before that it's swelling. A second session is discussed only after six months.
What that looks like for women from the UK and Ireland is on the country pages.
What a quote should contain
I give no prices here. Not because I don’t know them, but because any figure without an examination is misleading, and because this site also serves countries where price advertising for cosmetic surgery is restricted. A reputable quote for facial fat transfer is itemised by surgeon, anaesthetic, hospital stay, materials and aftercare, and it states what isn’t included. When two quotes differ widely, it’s almost always the surgeon’s experience, the materials, the hospital or the length of stay. I’ll walk you through the clinic’s actual quote line by line.
Common questions
How much fat stays?
Often half to two thirds. What's there at six months is largely permanent.
Is a second session needed?
Sometimes, and that's normal. Decided no earlier than six months.
How long will I look swollen?
Markedly for one to two weeks, with residual swelling for several more.
Fat or filler?
Fat is your own and permanent, but surgery. Fillers are quick and controllable but foreign and temporary, and no reason to travel.
How long in Turkey?
Usually one week. The surgeon clears the flight home.
What is nanofat?
Very finely processed fat to improve skin quality, not to add volume.
Will lumps remain?
Small hardenings are possible, especially in thin skin. Usually they settle.
What does it cost in Turkey?
It depends on extent and combination. I give no figures.
Can it be combined with a facelift?
Yes, the commonest and most sensible combination: lifting and filling complement each other.
What if I'm too slim?
Then there's no donor fat and this isn't the right method. Gaining weight for surgery is not a solution.
Let’s talk it through, without pressure
Twenty minutes, no obligation. We start by working out whether surgery is the right step for you at all, and whether now is the right time.
